Diabetes Self Management
Diabetes management establishes and coordinates, with assistance from the medical director, the clinical teaching/learning aspects of the diabetes program for patients and their families.
Certified diabetic educators who are registered nurses and dieticians act as liaisons between the hospital, physicians, patients and families.
There is an interdisciplinary approach to care, and the certified diabetic educator coordinates the care plan with team members. Inpatient assessment, blood glucose testing classes, nutritional counseling, and medication administration are available for all inpatients.
Outpatient services include a one-on-one assessment with a certified diabetic educator and classes that include stress management, medication (to include insulin administration), meal planning, blood glucose testing, foot care and exercise.
The Diabetes Self Management Program is certified by the American Diabetes Association. This certification ensures the continued delivery of high quality care to all inpatients and outpatients.
